Lash Extension Consent Form: What to Include (and Why It Protects You)
If you're doing lash extensions without a signed consent form, you're leaving yourself exposed. Not just legally — but professionally. A well-designed consent form signals to clients that you run a real business, not a side hobby. It sets expectations, documents allergies and sensitivities, and protects you if anything goes wrong.
The good news: you don't need a lawyer to put one together. You just need to know what to include.

What Every Lash Extension Consent Form Should Include
A solid consent form covers eight key areas:
Client name and contact information — Full legal name, phone number, and email. You'll need this for booking records and follow-ups.
Allergy history and known sensitivities — Ask specifically about adhesive allergies, latex sensitivities, and any reactions to beauty products. This is your first line of defense.
Relevant medical conditions — Thyroid disorders, alopecia, chemotherapy treatment, trichotillomania, and certain eye conditions can all affect lash retention or safety. Better to know upfront.
Previous lash extension history — Have they worn extensions before? Did they have any reactions or removal issues? This context matters more than most lash techs realize.
Aftercare acknowledgment — The client signs off that they understand and agree to follow aftercare instructions. If they ignore the rules and their lashes fall out at day 3, you have documentation.
Photo consent — A simple checkbox: “I give permission for before-and-after photos to be used for marketing purposes.” Easy to add, easy to forget, and very useful for building your portfolio.
Signature and date — No signature means no consent. Every appointment, every time.
Rebooking preferences (optional) — Some lash techs include this to streamline the booking conversation. Not essential, but a nice touch.

Common Mistakes Lash Techs Make With Consent Forms
Even lash techs who use consent forms often make these errors:
Relying on a verbal agreement. “She said she wasn’t allergic to anything” is not documentation. If something goes wrong, you have nothing in writing.
Using the same form for years without updating it. If you’ve added new services or switched adhesive brands, your form should reflect that. Review it annually.
Keeping paper forms with no backup. Paper gets lost, damaged, or thrown away. Scan and store digitally — or use a digital form from the start.
Skipping photo consent. You can’t use client photos for your Instagram without explicit permission. It’s a minor thing to add and a major thing to skip.
